MobiTest Solutions provides:

• On-site mobile drug and alcohol testing
   
• DOT and non-DOT compliant testing
   
• Medical courier services for clinics, labs, and healthcare providers
   

All services are subject to availability and service area coverage.

• Appointments can be scheduled through our website, by phone, or email.
   
• Cancellations must be made at least [24 hours] in advance. Late cancellations may incur a fee.
   
• “No-show” clients may be charged in full.
   

• Payment is due at the time of service unless otherwise agreed upon in writing.
   
• Accepted forms of payment include cash, credit/debit card, or business account invoice.
   
• Invoices must be paid within [30 days]. Late payments may result in a late fee of [X%] per month.
   

MobiTest Solutions follows DOT regulations for applicable testing. Clients are responsible for ensuring they meet DOT requirements for their business or industry.

• All items for transport must be properly labeled and packaged.
   
• We are not liable for delays caused by events outside of our control (e.g., traffic, weather).
   
• Hazardous or illegal materials are strictly prohibited.
   

You agree to:

• Provide accurate and up-to-date information
   
• Be present at the scheduled time and location
   
• Treat our staff with respect and professionalism
   

Failure to comply may result in termination of service.

To the fullest extent permitted by law, MobiTest Solutions is not liable for:

• Any indirect, incidental, or consequential damages
   
• Delays or service interruptions due to factors outside our control
   
• Errors resulting from inaccurate information provided by the client
